PRP services support the psychotherapeutic aspect of an individual’s mental health treatment by addressing what happens internally, mentally, and emotionally. Our focus on improving functioning despite a client’s mental health diagnosis is meant to shift from focusing on problems to focusing on increasing functioning. All our treatment approaches are evidence-based and include, but are not limited to, cognitive behavioral therapy, brief solution-based therapy, mindfulness and stress reduction, eye movement desensitization and reprocessing, and many others. All our treatment approaches utilize a person-centered, trauma-informed approach.
Functional Assessments

Supportive counseling directed at solving daily problems related to community living and interpersonal relationships
Psychoeducational groups pertaining to the alleviation and management of psychiatric disorders

Support with connecting individuals to natural community supports
Orientation to and assistance with accessing self-help and advocacy resources

Teaching of recovery skills to prevent relapse of mental health symptoms
At MNK Community Resources, we understand that every individuals path to wellness is unique. We create personal rehabilitation plans based on individualized mental, emotional and social needs. We service children (ages 6 and older), adolescents and adults.
Referrals and Eligibility
Referral forms must be completed by a licensed mental health clinician. Individuals with Medical Assistant (Medicaid, including PAC) are eligible for PRP services.

Intake
We begin by getting to know you. You will complete an confidential intake packet which will provide us not only with your demographic information but also provides you with your first opportunity to document what needs you have, your treatment goals and any symptoms you may be experiencing.
Identify
An Intake Specialist will review and discuss your intake packet with you to further identify your needs and to MNK Changing Lives has available for you. The Intake Specialist will also pair you with a Mental Health Associate who will assist you with reaching your treatment goals.
Rehabilitate
A Rehabilitation Coordinator will develop a treatment plan tailored to your needs. Together with our therapist, the mental health associate will be there to assist and guide you through your path to wellness.
Discharge
Clients meet discharge criteria when individual plan goals have been attained and when referrals to other support services completed as needed. Discharge may occur if the client no longer attends regularly or chooses not to actively participate in the program
